What Pivot does

Pivot Protocols is a behavioral consulting practice built around a specific population: people using kratom extracts, 7-OH products, full agonist opioids, or tapering off Suboxone who need a structured behavioral layer alongside their clinical care.

Pivot manages the non-clinical side — pattern assessment, VDI-guided stability monitoring, behavioral containment, and structured exit support. All clinical decisions remain between the client and their licensed medical provider.

Three referral pathways

Kratom extract, 7-OH, and opioid patients

The Kinetic Bridge is a virtual program built for compressed-cycle dependence produced by kratom extracts and 7-OH, and for full agonist opioid dependence. It uses short-term buprenorphine detoxification to overwrite the cycle pharmacokinetically within a defined clinical window. Standard induction protocols apply for full agonist patients.

Pivot provides education, preparation, and behavioral containment. The clinical work belongs with the prescriber.

Suboxone patients who want a structured exit

For patients stable on buprenorphine who want a sequenced path toward discontinuation, Suboxone Taper Consulting provides a VDI pattern assessment, a 60-minute Plan Review, and a written sequencing plan built around where the patient actually is. Taper decisions remain between the patient and their prescriber.

Recovery programs — the Kinetic Bridge

The Kinetic Bridge is a defined five-day pre-admission protocol that resolves the active compression cycle before a resident enters your program. The resident arrives on Day 6 pharmacologically clean, cycle overwritten, and ready to engage with recovery from Day One.

There is no cost to the program. The resident pays Pivot directly. Pivot prepares a plain-language transition summary for the resident to bring at admission.

For clinicians who want the full framework translation

The Clinicians and Researchers page maps each Pivot term to established clinical constructs, provides citation anchors, and documents where standard instruments meet their limits with this population — including the COWS timing gap and the emotional dimension of GBAD.
